The Social Worker (MSW) is the member of the interdisciplinary team whose primary focus is to assess the home care patient's psycho~social needs as related to their family background and environment as it impacts their medical condition and prognosis for positive outcomes.
Our Social Workers will:
Link clients with community resources
Provide information to different organizations for any practical or environmental needs the patient may have
